Registration Form   Open File

For mobile or desktop compatibility issues: please open the file using the free latest version of Adobe Acrobat Reader DC.

Recommended Browsers: Google Chrome, Mozilla Firefox and Safari  

STEP-BY-STEP INSTRUCTIONS:

1.) Fill out the form via internet browser. If not supported by your browser, open the file via Adobe Acrobat Reader DC.  

• Don't forget to fill-out the required fields up to the last page
• Please note that page buttons are not supported in some mobile devices

2.) After completing the required details, save the file by clicking the button as shown in the screenshot below.

• • Please fill out one form per child 

Save as PDF 

• If you're directly opening the file using Adobe Acrobat Reader, just click File > Save as > PDF...

3. Rename the file from SMS_Registration_Form_v1.pdf to SMS_Registration_Form_v1 (Dela Cruz, Juan).pdf 

    E.g.: SMS_Registration_Form_v3 (Dela Cruz, Juan).pdf  

4.) Now you can send the file to: This email address is being protected from spambots. You need JavaScript enabled to view it.

    Follow Email Subject format: SMS Forms Submission (LastName, FirstName)

    E.g.: SMS Forms Submission (Dela Cruz, Juan)
